A Certificate of Subsistence is a document that can be requested from the Department of State which will indicate the business status of the entity in question. Sometimes referred to as a certificate of good standing or certificate of existence, this document is most often used by businesses seeking registration in states/countries outside of their own jurisdiction. Requesting is simple and can be accomplished by completing the PDF application located below, attaching a $40 fee, and mailing both articles to the Department of State. How to Request
Step 1- Begin by downloading the Copy/Certificate Request application that the Department of State has provided.
Step 2- With the application downloaded, continue by submitting the following information:
- Return address (mail or email)
- 1-Name and entity number
- 2-Document and quantity being requested (subsistence certificate)
- 3-Method of payment
Step 3- Once this document has been completed, a filing fee must be prepared in the amount of $40. This fee should come in the form of a check or money order made payable to the Department of State. The application and fee must be mailed or delivered to the address below.
