This document is often required by foreign officials and financial institutions when applying for registration or requesting financing. Unfortunately there is no online method for filing, though obtaining this type of certification can be accomplished through the mail or by fax. A filing fee of $25 must be included by mail, and if sending by fax, payment must be made by credit or debit card along with the Credit/Debit Card Authorization form. How to File (Mail)
Step 1- To apply for a Certificate of Status by mail, a written request must be sent to the address below. With the request you must also include the filing fee of $25. Checks must be made out to the Department of State, or if paying by credit or debit, be sure to include Credit/Debit Card Authorization form with your filings.

How to File (FAX)
Step 1- Another option available to you for application would be to send a request by fax. Unfortunately cash and check can not be the payment of choice through this method, though credit and debit cards are accepted. An authorization form must be printed off and sent along with your request.
Step 2- Next, fill in the information specified below.
- Select the certificate of status option (fourth option from the top) and specify the type of processing)
- Choose your credit/debit card type
- Enter your card number and expiration date
- Name of card holder
- Billing address of cardholder
- City, state, and zip code
- Fax number
- Signature of cardholder and date signed
- If signing as a business entity or corporation, you must print the name of the person signing
Step 3- After completing the form above, it must be faxed to the number (518) 473-1654 along with your written request for a Certificate of Status.
