The tutorial on this webpage is here to provide information on how to obtain a Certificate of Status in the state of New York. This document is often required by foreign officials and financial institutions when applying for registration or requesting financing. Unfortunately there is no online method for filing, though obtaining this type of certification can be accomplished through the mail or by fax. A filing fee of $25 must be included by mail, and if sending by fax, payment must be made by credit or debit card along with the Credit/Debit Card Authorization form. Lets get started by scrolling down to the steps below.

How to File (Mail)

Step 1- To apply for a Certificate of Status by mail, a written request must be sent to the address below. With the request you must also include the filing fee of $25. Checks must be made out to the Department of State, or if paying by credit or debit, be sure to include Credit/Debit Card Authorization form with your filings.
